Support document package files

Currently if I have a MindNode file in a folder nvUltra will display it as a folder (which technically it is) instead of the "actual" document package abstraction it is. Also clicking on a link `[[Mind map.mindnode]]` to the package file doesn't open it, nvUltra tries to create a new note (I think).

  The next release will include file packages in the index, and not the embedded "subfiles".  It does mean that indexing will only work on the filename of the file (in other words, file packages are treated as binary files such as images).
