Keyboard Commands
CompletedAre there keyboard commands for the more esoteric Markdown markup like footnotes, blockquotes, code blocks, tables, etc.?
Where is a list of those?

For most of those, any keyboard command would be as long or longer than the actual Markdown syntax -- e.g. blockquotes simply use '>', so just type '>'..... ;)
For more complicated structures (e.g. lists), nvUltra automatically helps "do the right thing" most of the time, unless you disable those settings in the preferences.
If you have a more specific request/suggestion, we're happy to consider it.

The problem arises when you change your mind.
You write a section and then realize that you want it to be a list. Or a Numbered list. That is when the problem starts.
nvUltra is fantastic at handling the details in auto-numbering the content if you type it in. Or making lists, or designating blockquotes and codeblocks if you know what you are doing. When you change your mind, everything becomes menu driven.
In other words, the sub-items of the Convert Selection to menu would be improved by assigned keyboard commands. I have tackled that issue with Keyboard Maestro macros, but it might be better to build them in.
While we are on the topic, are footnotes completely manual?

After some discussion, Brett and I agree that for now:
1. It is unlikely that we would come up with a scheme of keyboard shortcuts that would suit the needs of majority of users of different languages/keyboards if we applied them to the majority of menu bar items.
2. macOS offers the means to customize keyboard shortcuts for menu bar items within System preferences, as well as with third party options (as you note).
3. I did go ahead and implement a feature that has been in the queue -- adding many of the core editing menus and submenus to the context menu within the text editor (the "right-click menu"). This is similar to what exists within MultiMarkdown Composer.
4. Footnotes are manual, but don't forget about inline footnotes `[^Inline footnotes allow you to create a footnote right inside the main text]`.
(As an aside -- there is a glitch in build 45 that makes typing inline footnotes impossible. Will fix for build 46, but for now you have to write the footnote, and then wrap it in `[...]`)

Gosh I had forgotten about inline footnotes as well thanks!
I have been training my writing team (marketing agency) in what markdown is (chuckle) and trying to wean them off Word. Footnotes are one of the biggest impetus for this.
However, as much as it is smoother to just write an inline footnote – more often they are importing a Word doc (no support for that here and I appreciate how troublesome that can be to get right seeing iA Writer, Typora and others struggle with the details), then reformatting the ordered list at the bottom.
For what it's worth, I really like a Drafts add-on implementation for this that:
1. wraps the footnote reference in [^n]
2. then auto adds a [^n]: n at the bottom of the document.
I assign it ⌃+⌘+F (same as iA Writer)
