Table with caption not recognized

If the caption of a table is placed in the preceding line of the table, nvUltra does not recognise table. Hence its background is not colored nor can cleanup be used.

  Correct.  MMD-6 requires captions to occur *after* the table.

  Yeah -- that one is *really* old.  (version 3 maybe???)
